This is an official Augusta National scorecard that has been autographed by 1946 Masters Champion Herman Keiser. Keizer's autograph is exceptional, and he added "1946 Champ." Herman Keiser passed away in 2003, but he lived in an assisted living facility for many years before then, which made his autograph nearly impossible to obtain late in his life.
