Offered here is a very cool item. This is Tiger Woods' actual application to compete in the 1999 US Open. Even though he was the most famous golfer on the planet, Tiger still had to send in an official application to compete in the US Open. The application was likely filled out by someone at his agency IMG, as Tiger's address is listed as their office in Cleveland, OH. The right side of the application required details of the applicant's finishes in local and sectional qualifying. Tiger, of course, was exempt from this ordeal so it is listed "Exempt S-3, S-9." Those two listed exemptions are for "Winner of the Masters Tournament past 5 years" and "Top 30 from 1998 PGA Tour Money List." The person who filled out this form for Tiger actually missed one exemption, as Tiger was also exempt under S-16 that year - "Top 20 from 1998 final World Rankings List." 

Tiger Woods has beautifully signed the bottom of this 1999 US Open application. Interior visible document is 8" x 10 3/4", and framed to an overall 13 1/2" x 16 1/8".
